  • Overview

    The Coordinator - Outreach develops and implements community collaborations, coordination and partnerships with targeted Nursing and IDD facilities, CBOs and state agencies in order to promote programs.  Project strategy is to educate and partner with appropriate CBO personnel in Managed Care Programs so they can assist low income families to help overcome barriers to utilization of available healthcare services. Position provides customer services via telephone or in person to clients and community members regarding the selection, enrollment, and eligibility requirements for various Texas Health and Social Services Programs. Makes home visits to LTSS clients upon request.


    • Conducts one-on-one and group outreach presentations.
    • Conducts public speaking and education of CBO staff.
    • Presents program education and enrollment options to recipients and agency staff.
    • Presents managed care options to recipients and assist them in using their health care plans. 
    • Maintains positive relationships with county staff, state agencies, community groups and recipients.
    • Completes direct data entry into designated internal systems in accordance with EB Project policies and procedures.
    • Maintains knowledge of all EB Project automated systems and training skills in order to support unit.
    • Conducts telephone calls and home visits in accordance with project policy and procedures.                                                                   
    • Knowledge of community and participates in community events, health fairs, and other activities as needed.
    • Responsible for reporting outcomes on a daily/ weekly basis.
    • Willingness to work a flexible schedule including weekends and evenings.
    • Performs other duties as may be assigned by management.

    The Ideal Candidate will Possess the Following Additional Education and Experience

    Must be able to cover:  Beaumont and Sourounding Counties:  Walker, San Jancito, Polk, Tyler, Jasper, Newton, Orange, Jefferson, Hardin, Chambers, and Liberty Counties.

    • High school diploma or GED; Bachelor’s degree in social work or related field preferred. Experience can be substituted for education.
    • 2 years of experience.
    • Must have knowledge of outreach.
    • Proficient in Microsoft Office products.
    • Knowledge of Excel preferred.
    • Ability to work with customers in a caring and respectful manner and with due understanding of and consideration for their unique circumstances. 
    • Excellent organizational, written, and verbal communication skills. 
    • Ability to perform comfortably in a fast-paced, deadline-oriented work environment. 
    • Ability to work as a team member, as well as independently.
    • Must be able to remain in a stationary position for an extended period of time.
    • Occasionally lifts, carries, or otherwise moves items weighing up to 25 pounds.
    • Work is often performed conducting home visits, presentations, and part in an office environment.
    • Knowledge of Long Term Services and Support for aged and disabled population.
    • Working with culturally and linguistically diverse, disadvantaged and aged/disabled populations, preferably specialized in long term services.
    • Highly developed problem solving skills.
    • Demonstrated ability to build high performing teams.
    • Knowledge of Medicaid preferred.
    • Preferred qualifications include six months of human services experience, dealing with the public, and commonly spoken language.
