About Children & Youth Evaluation ServicesThe Children and Youth Evaluation Service (C-YES) is a New York State Program that determines Home and Community Based Service/Level of Care (HCBS/LOC) eligibility for youth with medical, behavioral, and/or developmental needs aged 0-21 years. The C-YES program also provides Medicaid application assistance services to children who appear HCBS eligible and are either without Medicaid and/or have Medicaid but opt out of HH Care Management. C-YES also develops person-centered Plans of Care (POCs) and provides ongoing HCBS service coordination for those families who elect to remain with C-YES and opt out of more comprehensive case management provided by Health Home Care Management Staff. HCBS are meant to help youths remain in their community as opposed to going to a higher level of care (i.e., facility or institution). A Care Coordination team comprised of Nurse Evaluators and Family Support Coordinators are responsible for ongoing support to all families referred to C-YES. Position SummaryThe Family Support team is the ‘face’ of the Children Youth Evaluation Service (C-YES) for Help-line callers, consumers, State partners and referring providers. The Intake Family Support Coordinators will monitor the C-YES Helpline and provide answers to basic questions about the C-YES program, knowing when to take a message and escalate to a supervisor or the assigned FSC. The Intake Family Support Coordinators also manage all referrals received, initial outreach and scheduling, and assignment of consumers to Nurse Evaluators and Family Support Coordinators What You'll BringThe ideal candidate will have 1-3 years of experience working in a Call Center environment, excellent verbal communication skills and a passion to provide superior customer service.; ability to learn new software programs quickly and intuitively; excellent organizational, interpersonal, written, and verbal communication skills; ability to perform comfortably in a fast-paced, deadline-oriented work environment; ability to successfully execute many complex tasks simultaneously; and ability to work as a team member, as well as independently. Pay TransparencyMaximus compensation is based on various factors including but not limited to a candidate’s education, training, experience, expected quality and quantity of work, required travel (if any), external market and internal value analysis including seniority and merit systems, as well as internal pay alignment. The SOA PMO team is responsible for all new government contracts and implementations across the enterprise. Project managers who have experience with government contracting and/or Medicaid/Medicare is a plus. This is a remote position with travel requirements of up to 25% that may be required to project sites during implementations. Please note, an active PMP certification is required for this role.

It provides services directly and through a network of 7 Care Coordination Organization, with about 80 percent of services provided by the private nonprofits and 20 percent provided by state-run services. About the PositionWe’re seeking to hire 2 Operations Support Coordinators for the OPWDD Project. The Operations Support Coordinator will be responsible for starting the Coordinated Assessment System (CAS) notification process by contacting Care Coordination Organization or ICFs to confirm and gather necessary information to generate notification notices to all involved partied such as individuals, Care Managers, IRA manager and actively involved family member.
